INDIVIDUAL COUNSELING
Each client is assigned a primary Caseworker who will act as case manager, with responsibility for managing the case and monitoring the client's progress at IMPACT and through aftercare. Caseworkers are assigned by the program coordinator according to client age, gender, ethnicity and special needs. Caseworkers present cases they manage during weekly case conferences, and are responsible for maintaining comprehensive records on services provided and other relevant information in the treatment record. The Caseworker keeps track of, and follow up on,
- All referrals for adjunct services outside IMPACT.
- An initial treatment plan is developed for all clients by the Caseworker in conjunction with the client at the onset of admission.
- A Master Treatment Plan will be developed when client assessments have concluded.
- All clients are reviewed in weekly case conference after completion of the Master Treatment Plan and every two weeks thereafter to ensure their adjustment to the treatment process.
- After 60 days in treatment, cases will be reviewed every 30 days.
Case conference is attended by Caseworkers, Program Coordinators, the Clinical Director and other staff with direct client contact. In addition, clients are evaluated on an "as needed" basis in weekly special interest case meetings. Phase changes, disciplinary actions, special treatment plans, and discharges are determined in these meetings.
IMPACT's comprehensive client charting system includes notations on individual
- sessions
- progress notes and rating
- health, appearance
- attitude
- responsibility
- relationship, daily participation
- indirect service record
- referrals and monthly status reports
The primary Caseworker is responsible for keeping the chart current and the records coordinator reviews the chart weekly and makes recommendations to the Caseworker, as indicated. Information is used in treatment planning and is used to help determine whether clients are responding satisfactorily to treatment. IMPACT also uses it's information management system to track client progress through individual treatment plans and as they transfer in and out of the comprehensive treatment system
